How to use the online form
If you'd rather fill out fields than compose an email, the online form is at /request. It takes about 2 minutes and ends up in the same inbox as an email — no advantage either way for the business outcome.
When the form is better than email
- You want to make sure you don't forget any details
- You want to attach multiple files cleanly
- You're submitting on behalf of someone else and want a record of exactly what you sent
Walk-through
Step 1 · Are you a new or existing vendor?
The first screen asks "Have you worked with Modal before?"
- New / one-off: Click New vendor and skip ahead to fill out the form from scratch.
- Existing: Click Existing vendor and you'll be asked for a PIN. The PIN unlocks pre-filled templates from your past shipments — much faster. See Existing vendor? Use your PIN.
Step 2 · Fill in the request
The form is split into sections:
- Your info — name, company, email, phone (so we can reach you back)
- Pickup — address, time window, contact at the pickup location
- Delivery — same fields for the destination
- Cargo — pieces, weight, dimensions, temperature class
- Special handling — checkboxes (TSA-screened, pharma, dry ice, etc.)
- Reference numbers — AWB, BOL, PO
- Attachments — drag-drop or click to add. Up to 50 MB total.
Required fields are marked with a red asterisk. Everything else is optional.
Step 3 · Review before submit
The last screen shows everything you entered, read-only. Look it over, hit Edit to fix anything, or Submit when it's right.
Step 4 · Confirmation page
After submit, you'll see a confirmation page with a tracking ID. Save the URL — you can use it later to refer to this specific request.
You'll also get a confirmation email at the address you entered.
What attachments work
- PDF (any kind)
- Word (
.docx,.doc) - Excel (
.xlsx,.xls,.csv) - Images (JPG, PNG, HEIC)
Up to 25 MB per file, 50 MB total, max 10 attachments per submission.
What if the submit button isn't working?
Most likely you missed a required field — scroll up to find the red asterisks. If everything looks complete and it still won't submit, refresh the page and try again. Cloudflare (our bot protection) sometimes blocks browsers that look automated; try a different browser as a workaround. If it keeps happening, email us instead.
